I am reporting a severe, systemic vehicle performance discrepancy on my Lexus TX 500h (2.4L Turbo Hybrid powertrain). The vehicle is officially labeled with an EPA rating of 27 MPG combined. However, under normal, conservative driving conditions, the vehicle routinely achieves only 20 to 21 MPG—a consistent 20%+ shortfall from the manufacturer's certified window sticker metrics. This has drastically limited the vehicle's safe total driving range to under 300 miles per tank. The shortfall persists despite cautious driving in Eco Mode and after passing the engine break-in period. This issue matches a widespread pattern reported by numerous other owners in consumer forums, pointing to a potential defect in the vehicle’s fuel management software or hybrid battery management logic. I request that NHTSA log this tracking data to investigate whether the manufacturer misrepresented the vehicle's real-world operational capabilities or fuel-system integration.

Severe safety issue! Pulled the vehicle into the driveway after a drive and put the vehicle in park and turned the car off. With key still in the vehicle, exited vehicle, closed driver door and went to open drivers-side rear passenger door where 2 year old baby was in car seat. Before being able to open the rear door, the car went dead and locked down all the doors (key is still inside the car). Could not get into car through any of the doors, including rear hatch. Tried the manual back up door handles (since this vehicle has electronic door releases), but none of them worked. All the while, baby is crying in the car and it's more than 85F outside. In panic, call Lexus roadside assistance. They advise that help may take 90 minutes and with baby in vehicle, we should call 911. After call to 911, fire and police arrive. Fireman tries all manual backup door handles (which are there in case battery dies and electronic release don't work) then takes crowbar to the door (bent door frame and A pillar, and other damage to car). Door still not open! Door is later opened by service tech and we successfully get baby out. Had another incident where after drive, put vehicle in park and turned off vehicle. The vehicle power immediately went dead (no lights, no ignition, nothing) and I couldn't exit car even when using the manual door handles. Had to sit and wait in car for a while until power came back to get out. This is extremely dangerous because someone could get stuck inside the car in traffic or somewhere else and be in death box unable to exit. Or someone, including kids, could get locked without. The physical key inserted under the door handle tab and using the manual override handles still doesn't work - it's an extremely dangerous design flaw. There was a police report made by Franklin Park, Illinois PD. We don't have a copy of the report yet.

The contact owns a 2024 Lexus TX. The contact stated that while driving at 41 MPH, a vehicle ran a stop sign while distracted and hit the contact's vehicle. The contact was able to swerve and avoid a head-on collision. The contact's vehicle was struck on the right driver's side toward the rear bumper. The vehicle was pushed off the road, where it came to a stop. The driver-side and passenger-side air bags did not deploy. The driver-side and passenger-side air bags did not deploy. The driver-side seatbelt did not lock as intended. From the impact, the contact was propelled over the center console. The contact was taken to the hospital by ambulance. The contact suffered a concussion and sustained injuries to the neck, both knees, hip, her back, and right foot. The contact was scheduled for physical therapy and suffers from post-concussion syndrome. A police report was filed. There were no passengers in the vehicle. The vehicle was towed to Mayfield Wrecker Service, and then it was towed to the dealer. The cosmetic part was repaired on the vehicle. However, the safety issues were not repaired. The vehicle is at the residence. The insurance company was contacted. The contact related the failure of the side curtain air bags to NHTSA Campaign Number: 24V461000 (Air Bag). However, the VIN was not included. The manufacturer was not made aware of the failure. The failure mileage was approximately 18,000.

On two occasions while driving my hood has become open due to windy conditions coming from the Makua side of the highway as I’m driving on the Queen Kaahumanu Highway Today was the second incident except that I was heading south The first time that happened I was driving north and it occurred probably about a few months ago, but I dismissed it I know for a fact that I did not open the engine compartment I will download my front camera files and see if I can find a record

The contact owns a 2024 Lexus TX 500H. The contact stated that while driving approximately 50 MPH, the vehicle drove over glass in the roadway that struck the air conditioner condenser. Approximately two weeks later, the air conditioner failed to blow cool air. The vehicle was taken to a local dealer, where it was determined that the A/C condenser was damaged by the debris, causing a coolant leak. The dealer stated that the condenser needed to be replaced. The contact associated the failure with NHTSA Action Number: DP26006 (Engine and Engine Cooling). The manufacturer was notified of the failure. The failure mileage was 37,000.
