I have never been in an accident in my 31 years of driving. However, I am worried that this van is going to cause me to be hit. Several times I have pulled out to make a left or right turn and my gas pedal does NOTHING. I can push it clear to the floor and NOTHING!!! Cars end up slamming on their brakes and honking at me but there's nothing I can do. This is EXTREMELY scary and a HUGE HUGE HUGE manufacturing flaw. There needs to be a recall for this to be fixed before people are injured.

Rear Tail Lights Failed... Brake and Signals work. This was repaired through the dealership. 4 connectors and housing replaced invoice shows part 909890-10942. I am writing to express my concern regarding repeated tail light failures on my black Toyota Sienna that appear to be caused by wiring connection issues. This defect creates a serious safety hazard for my family, other motorists, and pedestrians, particularly in rural areas where there is little to no street lighting. I live in the country, where many roads are unlit and visibility at night is already limited. Because my vehicle is black, it blends into the darkness when the tail lights fail. Drivers approaching from behind may not realize my vehicle is on the road until it is too late to stop safely, significantly increasing the risk of rear-end collisions. This issue is especially concerning because I frequently transport children in my vehicle. Their safety depends on other drivers being able to clearly see my vehicle at night. A tail light failure caused by a wiring defect could place them at unnecessary risk of serious injury or death if another vehicle is unable to see us in time. The defect also puts other drivers at risk. Motorists traveling at highway speeds on dark rural roads rely on tail lights to judge the presence, distance, and speed of vehicles ahead. Without functioning tail lights, they may have little or no warning that another vehicle is in their lane, particularly around curves, hills, or during poor weather conditions. This appears to be more than a routine bulb replacement issue. If the root cause is an unreliable wiring connection, the lights may work intermittently or fail unexpectedly, making it difficult for owners to recognize the problem before driving. An intermittent electrical failure is especially dangerous because drivers may believe their lights are operating normally when they are not. Started a case with Toyota 260715001670

Three (3) separate systems failed. 1. Upon seeing cars ahead of us slowing down, my husband pressed the breaks to slow down to avoid collision. When he pressed on the breaks, instead of slowing down or stopping the vehicle, the brakes failed to engage, and caused a surge that compelled the vehicle to lurch forward. The lurch propelled our vehicle into collision with another vehicle. 2. Also, the forward collision warning system failed to alert or engage the breaks. Our vehicle was stopped by the impact of hitting the other vehicle, not by its own breaks. 3. The automatic breaking system also failed to engage at all prior to impact.

The contact owns a 2024 Toyota Sienna. The contact stated that upon starting the vehicle while parked in the garage, the low tire message was displayed. The contact stated that the tire was reinflated. The vehicle was taken to the local dealer and the contact was informed that the issue might be weather-related. There was no repair performed. The manufacturer was contacted, but no assistance was provided. The failure mileage was approximately 17,826.

My child [XXX] ) engaged the automatic locking retractor (ALR) and as it wound back in it wrapped around his neck. By the time we got to him it was tight enough that it was difficult to move or rotate or slide the seatbelt over his head, and we ultimately cut the seatbelt to free him. In addition to the actual safety incident, I was very surprised to learn that this was even possible. I had never heard of this safety issue, and had absolutely no game plan or awareness about this issue. I saw the SBIR program addressing innovation in ALR systems which is exciting! However I would also strongly encourage some sort of awareness campaign or warning messaging to raise awareness and ensure parents keep belt cutters in their cars. The contact owns a 2024 Toyota Sienna Hybrid. The contact stated that while driving at an undisclosed speed, the hatchback independently opened. No warning lights were illuminated. The vehicle was taken to a dealer where it was diagnosed and determined that the hatchback needed to be recalibrated. The vehicle was repaired; however, the failure persisted. The vehicle was taken back to the dealer where it was diagnosed and determined that the hatchback needed to be recalibrated. The vehicle was repaired; however, the failure persisted. The vehicle was taken back to the dealer again and was diagnosed and determined that the hatchback needed to be recalibrated. The vehicle was repaired; however, the failure persisted. The manufacturer was notified of the failure. The failure mileage was approximately 15,000.
